The question started with my daughter. She had glandular fever โ Epstein-Barr โ and it had inflamed her liver, pushing her liver enzymes up. Suddenly the most ordinary thing in the medicine cabinet, acetaminophen โ paracetamol, in Europe โ was not automatically safe. It had to be used carefully, or avoided. That stopped me short. If the safest painkiller we own can turn into something you have to think twice about, what exactly are these things doing when we swallow them without a second thought?
I had stopped taking painkillers myself back in early 2020. So I went looking for what the evidence actually said I had avoided. The answer surprised me โ and it is not the answer the headlines or the supplement blogs would give you.
A confession up front
I stopped taking painkillers in early 2020. Acetaminophen, ibuprofen โ I simply stopped reaching for them. With one exception: a while later I cracked something in my rib cage, an intercostal muscle strain that made breathing hurt, and I took ibuprofen for four days straight until the worst passed.
Here is the honest part, and it is the foundation of everything that follows. Those four days did me no harm, and the evidence is clear that they didn’t. A short course of over-the-counter ibuprofen in an otherwise healthy adult is not where the danger lives. Indeed, the European Medicines Agency states plainly that there is no increased cardiovascular risk with ibuprofen at doses up to 1,200 mg a day โ the ceiling for over-the-counter use โ and virtually every study showing harm is looking at people who take these drugs regularly, for months or years, and often at higher doses.
So this is not an article telling you that the occasional pill is secretly poisoning you. It isn’t. This is an article about the difference between occasional and constant โ a difference that turns out to be almost everything, and one that disappears from nearly every piece you’ll read on the subject.
What acetaminophen actually does
Acetaminophen โ sold as Tylenol in the US, and known as paracetamol across Europe โ is processed by your liver. About 90% of it is broken down harmlessly. But a small fraction, less than 5%, goes down a different path and becomes a reactive, toxic compound called NAPQI. Your liver neutralizes it instantly using glutathione, your body’s master antioxidant, and clears it away.
At a normal dose, this is a non-event. The toxic sliver is tiny, glutathione mops it up, and afterwards your liver replenishes what it used between doses. Liver damage only begins once glutathione is more than about two-thirds depleted โ and consequently that essentially only happens with overdose, or when your reserves are already low from heavy alcohol use, fasting, or illness.
You may have heard that acetaminophen is the leading cause of acute liver failure. That’s true โ indeed, in the United States it accounts for roughly 46% of cases. But read that carefully: those are overdoses, whether deliberate or accidental, with a median intake equivalent to dozens of tablets at once. It is not, therefore, what happens to someone taking the labeled dose for a headache.
There is one finding worth flagging for regular users, though, because it comes from a proper randomized trial rather than an association study. In people who already have high blood pressure, taking 4 g of acetaminophen a day โ the maximum dose โ for two weeks raised their systolic blood pressure by about 4.7 mmHg compared to placebo. That’s the PATH-BP trial, and it’s solid. It doesn’t matter if you take acetaminophen twice a year. It matters a great deal if you have hypertension and take it daily.
The margin nobody mentions
Here’s the part that genuinely changed how I think about acetaminophen, and it runs against every instinct. Acetaminophen feels like the gentle option โ the one you give children, the one that doesn’t upset your stomach. But it has a strikingly narrow safety margin. The toxic dose of acetaminophen is only about 10 times the therapeutic dose. For ibuprofen, it’s roughly 60 times.
Put concretely: the label says 4 g a day is fine. At 8 g โ just twice that โ your liver can fail and you can die. By contrast, a fatal ibuprofen overdose is remarkably hard to achieve; indeed, one review of the entire medical literature up to 2006 found just ten documented cases in history, and nine of those involved complicating factors. The tenth, meanwhile, was someone who took the equivalent of over 500 tablets.
This margin is so tight that pharmacologists have said openly that if acetaminophen were discovered today, regulators would almost certainly not wave it through for over-the-counter sale as freely as it is now โ the liver risk would trigger tighter restrictions. It’s grandfathered in by a century of use, not by the safety profile a modern approval would demand. That’s not a reason for panic at normal doses. It’s a reason to respect the ceiling far more than most people do โ especially since the drug hides in so many combination cold-and-flu products that going over 4 g without noticing is the single most common route to accidental overdose.
What NSAIDs actually do
First, a definition, because the term gets used without explanation. NSAID stands for Non-Steroidal Anti-Inflammatory Drug. It’s the umbrella name for anti-inflammatory painkillers that aren’t steroids โ ibuprofen, naproxen, diclofenac, aspirin. They work by blocking an enzyme called COX, which dials down inflammation and pain. The catch is that the same enzyme also protects your stomach lining and helps regulate blood flow to your kidneys. Block it everywhere, and that’s where the side effects come from. Acetaminophen, worth noting, is not an NSAID โ it works differently and carries a different risk profile, which is why the two deserve separate treatment.
NSAIDs cause trouble in three places:
Three organs, three risks
The gut. By suppressing those protective prostaglandins, NSAIDs leave the stomach lining vulnerable to erosion, ulcers and bleeding. Regular users carry roughly four times the risk of an upper gastrointestinal bleed compared to non-users, and ulcers develop in 1 to 4% of long-term users. That risk multiplies sharply with age, a history of ulcers, or the concurrent use of blood thinners, steroids or certain antidepressants.
Here’s the part that unsettled me, and it’s why this makes it onto public health broadcasts. The damage is often silent. In fact, up to 70% of long-term NSAID users show stomach damage with no warning symptoms at all โ no pain, no warning, until a bleed announces itself. You cannot, therefore, rely on how you feel to tell you your stomach is coping. One redeeming detail: ibuprofen is actually the gentlest of the common NSAIDs on the stomach โ its relative risk of a serious upper GI bleed is about 2.7, versus 4.0 for diclofenac and 5.6 for naproxen. So if you must take an NSAID occasionally, ibuprofen at a low dose is the least aggressive choice.
Kidneys and heart
The kidneys. Furthermore, NSAIDs constrict the blood vessels that feed the kidneys, reducing their blood supply. In older adults, as a result, this roughly doubles the relative risk of acute kidney injury โ though the absolute risk over any short period stays low.
The heart. A large meta-analysis of randomized trials found that high-dose NSAIDs raise the risk of serious vascular events โ heart attacks, strokes โ by about a third. Interestingly, naproxen is the exception; it doesn’t appear to carry the same cardiovascular signal, though it has a rougher stomach profile in return. This isn’t fringe science: in October 2024, Japan’s health ministry formally ordered heart attack and stroke to be added to the warnings on NSAID labels.
The triple whammy
And then there’s the combination that quietly does the most damage. It’s known as the triple whammy: an NSAID taken alongside a blood-pressure pill (an ACE inhibitor or ARB) and a diuretic. Each is common in people over fifty. Together they can gang up on the kidneys, and the acute kidney injury risk jumps โ most sharply in the first month. If you take medication for blood pressure, an NSAID is not a casual choice.
The distinction almost everyone drops
Here is the pattern you may have noticed running through all of the above. The gut bleeds, the kidney injuries, the cardiovascular events, the blood-pressure rise โ almost every one of these findings comes from studying regular, long-term, or high-dose use. Usually in older people, usually with other conditions and other medications in the mix.
In absolute terms, moreover, the effects are often small. High-dose NSAIDs add roughly three serious vascular events per 1,000 people treated for a year. Similarly, the acute kidney injury signal in older new users works out to a fraction of a percent over a month. These are real, and they compound โ but they are not the picture of a single tablet doing catastrophic harm.
The danger isn’t really the pill. It’s the pattern. Reaching for something day after day, out of habit, to paper over a headache or an ache that keeps coming back โ and doing it as you get older, when your liver clears drugs more slowly, your kidneys have less reserve, your gut lining is thinner, and you’re more likely to be on the very medications that turn an NSAID into a triple whammy. Every one of those age-related shifts amplifies the harm.
This is the distinction that vanishes from nearly everything written about painkillers. The alarming version says “these drugs damage your organs.” The reassuring version says “they’re perfectly safe, they’re over-the-counter.” Both are wrong, because both skip the only variable that matters: how often, how much, for how long, and who you are when you take them.
A figure worth correcting
While I’m on the subject of getting things right โ I’ll correct a figure I used to repeat myself. You’ll often see the claim that “NSAIDs are responsible for 30% of all reported side effects.” That’s a misquote. The real figure, from a 2016 British Journal of General Practice editorial, is that NSAIDs account for around 30% of hospital admissions due to adverse drug reactions โ a much narrower and more specific thing than “all side effects.” The number is still striking. But precision is the whole point of this series, and a stat that falls apart under a fact-check does more harm than good.
The children question
This is where the narrow margin stops being abstract, and it’s the thread that started this whole article โ my daughter, a liver, a drug I’d always assumed was harmless.
Acetaminophen is the go-to for children precisely because it’s considered the safe one, and used correctly it is. But children are dosed by weight, in milliliters of syrup rather than tablets, which introduces a failure mode adults don’t face: the arithmetic. A tired parent, an unclear label, two caregivers who each give a dose, a switch between strengths โ and the gap between “right dose” and “too much” is far smaller than for almost any other medicine in the house.
The numbers are sobering. A review of 200 reported fatal adverse reactions to acetaminophen in children under five found that 30% involved a medication error โ for instance accidental overdose by a parent or health professional, a child getting into the bottle, or the wrong product given. Half of those error cases, moreover, involved liver damage. This isn’t the drug behaving unpredictably; instead, it’s the drug behaving exactly as its chemistry dictates, in a situation where a small mistake has no margin to absorb it.
None of this means acetaminophen is wrong for a sick child. It remains the first-line choice, and untreated fever carries its own risks. It means the ceiling deserves genuine respect: know the weight-based dose, write down every dose and the time you gave it, never combine two products that both contain acetaminophen, and if you’re ever unsure whether a child has had too much, treat it as urgent rather than waiting for symptoms. The warning signs of liver injury are late, not early.
Why NAC won’t save you from your pills
This section is one I have to be honest about, because I take NAC myself, and the reasoning that might tempt you to take it for this purpose is reasoning I had to talk myself out of.
The logic runs like this: acetaminophen burns through glutathione. NAC and glycine are the building blocks your body uses to make glutathione. So topping them up should protect your liver, right?
Steps one and two are correct. The problem, however, is the hidden assumption in step three โ that glutathione is the bottleneck. At a normal acetaminophen dose, it isn’t. Only that tiny toxic fraction touches glutathione at all; your liver clears it without strain, and afterwards it rebuilds what it spent. Your glutathione dips slightly and then recovers. There’s no shortage to top up. Supplying extra raw material to a factory that isn’t short on raw material doesn’t speed anything up.
Think of it as a jerrycan of fuel in your boot. It does nothing on a normal drive โ your tank isn’t empty. It only matters when you’ve run dry. And with acetaminophen, you only run dry in overdose.
Where NAC does work โ and where it doesn’t
Which is exactly where NAC earns its reputation โ spectacularly. In an overdose, the normal pathways are overwhelmed, far more of the drug goes down the toxic route, and glutathione genuinely runs out. Now it is the bottleneck, and NAC is life-saving. It’s been the standard antidote since the 1970s, and given in time it’s almost completely protective. The jerrycan, at the moment the tank hits empty.
The chronic case, where it gets strange
But here’s the part that surprised me, and it’s the one worth sitting with. What about the in-between case โ someone chronically taking too much, steadily over the limit? You’d assume depleted glutathione, so NAC helps. However, the body does something counter-intuitive: under sustained heavy load, it ramps its own glutathione production up to keep pace. In those people, therefore, production isn’t the bottleneck either โ it’s already running flat out. And crucially, there is no study showing that daily NAC or glycine protects chronic acetaminophen users from liver damage. Not disproven โ never properly tested. Which is precisely the kind of gap a supplement seller fills with a story that sounds plausible and has never been checked.
So the honest conclusion is sharper than “it doesn’t work.” If you’re taking enough painkillers to genuinely deplete your glutathione, the answer isn’t a supplement on top โ it’s addressing whatever is making you need that many painkillers. Adding NAC so you can keep over-using is mopping the floor with the tap still running.
(To be clear about my own stack: I take NAC and glycine for reasons that have nothing to do with acetaminophen โ the broader glutathione-and-ageing research, which is genuinely interesting and genuinely preliminary. That’s a separate article, with its own evidence and its own caveats. It just isn’t a reason to reach for NAC as a shield against the pills in your cabinet.)
What I do instead
I won’t pretend I never get a headache. However, since I stopped reaching for painkillers, I’ve found most everyday aches resolve on their own faster than I expected โ especially once I dealt with what was actually causing them.
The honest ranking of the alternatives, by how well the evidence supports them:
- Magnesium, for migraine โ the best-supported. This one has real randomized-trial backing. A classic trial found 600 mg of magnesium a day cut migraine frequency by around 42% over twelve weeks. Note the specificity: this is migraine, not ordinary tension headache, and the main side effect is loose stools.
- Hydration and sleep โ plausible but weaker. Dehydration and sleep debt are genuine headache triggers, and fixing them cleared most of mine. But the trial evidence here is thin, so I’ll call it what it is: it works for me, it makes mechanistic sense, and it isn’t strongly proven.
- Waiting it out, for the rare bad one. Not stoicism โ a risk calculation. For me, the temporary discomfort is a better deal than habitual use.
And one hard line worth knowing, because it’s the point where “occasional” tips into “problem.” Taking any painkiller on 10 to 15 or more days a month, for more than three months, can cause medication overuse headache โ headaches driven by the very drugs meant to treat them. Occasional use doesn’t do this. The threshold is structural, regular use over months.

No. A short course of over-the-counter ibuprofen in a healthy adult with no relevant heart, kidney or stomach conditions and no interacting medication is not where the risk lives. The sensible advice โ lowest effective dose, shortest duration, with food โ is prudence, not a sign of harm done.
Is acetaminophen safe if my liver is inflamed?
It’s more nuanced than “forbidden.” In liver conditions, acetaminophen is usually still preferred over NSAIDs โ but at a reduced maximum dose, typically 2 g a day rather than 4, and without alcohol. NSAIDs are harder on the gut, kidneys and heart, especially alongside blood-pressure medication. For most healthy people taking something once in a while, both are fine at label doses.
Should I take NAC to protect my liver?
Not for everyday acetaminophen use โ there’s no evidence it helps when your glutathione isn’t the bottleneck, which it isn’t at normal doses. NAC is a proven antidote for overdose, a hospital treatment, not a daily shield.
The honest summary
Painkillers are so normalized that questioning them feels almost radical, and the reflex is to answer in extremes โ either they’re quietly destroying your organs or they’re harmless because you can buy them in a supermarket. The truth sits in the variable both extremes ignore: dose, frequency, duration, and who you are.
For me, stopping was easy, and I don’t regret it. Nevertheless, the honest version of my own story is that the four days of ibuprofen cost me nothing, and the real lesson was never “pills are bad.” Instead, it was that the gap between the occasional tablet and the daily habit is the whole story โ and it’s the part that keeps getting left out.
